Crews Lake Middle School offers the following sports: Football, Volleyball, Basketball, Soccer, Cheerleading and Track. 6th-8th grade students are welcome to tryout.

CLMS 2020-2021 Athletics Update and Tryout Info

Football
Athletic Participation Packets must be turned in to Coach Petrucci by September 4th.  Tryouts will be held on Tuesday, September 8th through Friday, September 11th from 3:30pm-6pm.  Athletes are to meet in the gymnasium.  Parents should pick up their child at 6:15pm in front of the gym.  Items for tryouts will include shirt, shorts, sneakers, rubber cleats, any medical items, and water bottles or drinks.

Cheerleading

Volleyball
Athletic Participation Packets must be turned in to Coach Petrucci by September 4th in order to tryout.  Tryouts will be held at 6:30AM in the gym.
8th grade tryouts – Wednesday September 9th
7th grade tryouts – Thursday, September 10th
6th grade tryouts – Friday, September 11th

The Links below are for the Athletic Participation Packet and Athletic Fees for sports at Crews Lake Middle School.  Physicals must be completed before the first day of pre-conditioning or tryouts for all sports.  All forms must be completed FULLY.  We have a new form this year, the COVID-19 waiver which must be completed as well.  Below are the forms, number of pages, and how it must be completed.

  1. EL2 (Physical Evaluation) – 3 pages – Completed by doctor, parent, and student
  2. EL3 (Consent and Release from Liability) – 4 pages – completed by parent and student
  3. Pasco County Schools Athletic Information – 1 page – completed by parent and student
  4. Athletic Participation Form – 1 page – completed by parent and student and notarized
  5. Assumption of Risk and Waiver – 2 pages – completed by parent and student
  6. Parent Release – 1 page – completed by parent and student

Tryout Dates:
August 6th – 8th grade students 2:30pm-5:00pm
August 7th – 7th grade students 2:30pm-5:00pm
August 8th – 6th grade students 2:30pm-5:00pm
August 12th – Make-up tryout session, after school until 5pm
